Required Information

This information is required for correct order submission:

  • Date of order

  • Patient’s last name, first name and middle initial

  • Patient’s date of birth

  • CPT code of test or procedure, including description

  • ICD-10 code, including description

  • Name and signature of provider creating order*

  • Insurance information

  • Guarantor for minors

* Signature stamps or nurse’s signature on orders are not accepted. The ordering provider (doctor, nurse practitioner, physician assistant, etc.) must sign.

Additional Information

This information is preferred, but not required:

  • Demographic information (address, phone number, etc.)

  • Primary care physician

  • Authorization number
