Advance Directive

An advance directive helps your loved ones make decisions for your care when you can't
An advance health care directive, sometimes called a “living will,” is a legal document that specifies what actions to take for your care if you are no longer able to make decisions for yourself due to illness or incapacity.
If you are unable to make your own health care decisions, due to illness or incapacity, your loved ones can use your advance directive to ensure they are following your wishes for your care. An advance directive can be an Individual Instruction or a Power of Attorney for Health Care.
