Care at Home

Sometimes, you and your care team determine that it's time for you to go home, but you still need some additional support. These services will help you continue to recover while reducing the need to go back to the hospital.

End-of-Life Care

At the end of life, we understand the importance of managing symptoms, keeping you comfortable and honoring your wishes. These services all work in conjunction to make difficult days easier for you and your family.

  • Home hospice  provides comfort care at home, supporting family caregivers.

  • Inpatient hospice  provides end-of-life care for those too frail to transfer home.

Palliative Care Options

Palliative care focuses on symptom relief and support for patients facing serious illnesses such as cancer, heart failure and COPD. It is open to patients in restorative care and hospice care. 
Palliative care is offered in addition to care you already receiving from primary care provider and specialists. You may have seen a palliative care specialist while a patient at NMMC-Tupelo. 
NMMC Palliative Care offers outpatient care for:
 

  • Discharged palliative care patients for follow-up care through home visits

  • Cancer patients at NMMC Hematology Oncology in Tupelo

  • Heart failure patients in coordination with Cardiology Associates
