Baldwyn Nursing Facility is home to 107 residents who require long-term or short-term specialized care. We have a long tradition of excellence, serving this area since 1987. Baldwyn Nursing Facility is affiliated with North Mississippi Medical Center in Tupelo, a 2006 recipient of the Malcolm Baldrige National Quality Award and North Mississippi Health Services, a 2012 Baldrige Award recipient.

Resident care is directed by a licensed physician. Our nursing staff includes a certified nurse practitioner, RNs, LPNs and certified nursing assistants who provide loving care to Baldwyn Nursing Facility residents. Respiratory therapists also provide 24-hour service.
The social work and activity staff provide leisure opportunities for residents, including Bingo, crafts and exercise sessions.
Rehabilitation Services are provided from 7 a.m. to 4:30 p.m. seven days each week for inpatient treatment at Baldwyn Nursing Facility.
We are equipped to treat a wide variety of diagnoses from orthopedic injuries to complicated medical and neurological disorders. Our staff has extensive knowledge in physical, occupational and speech/language rehabilitation treatment techniques. We work closely with the medical team, patients, and families to provide education and training prior to discharge to allow for a smooth transition home or to an alternative discharge environment.
Upon referral from a physician, a speech, occupational and/or physical therapy assessment is performed and a plan of care for rehab is established. Our areas of expertise include treatment of the following conditions:

Treatment duration depends on the severity of the injury/dysfunction and the patient’s level of progress. To be admitted to the short-term rehabilitation unit, the patient must have experienced a prior qualifying hospital stay of at least three nights. The patient must have a chest X-ray and tuberculosis (TB) skin test prior to transfer. The patient must be medically stable and require skilled care. Direct referrals from home, hospital or another health care facility can be made through a case manager or by calling our nursing facility social worker at (662) 365-4083.
If it is recommended by the rehabilitation physician that the patient receive therapy, the patient will be expected to participate in therapy sessions and ask questions. As the patient gets stronger, he or she will be encouraged to do as much as possible independently to better prepare for return home.
Social visits to the patient in the gym are discouraged as this compromises other patients’ privacy. Family training before a therapeutic pass and before discharge will be arranged by the therapy staff.
Therapeutic Leave
The Therapeutic Leave program allows patients to leave the inpatient facility on a day pass (not to exceed eight hours). The purpose of therapeutic leave is to enable patients to feel comfortable in their own surroundings and help them adapt to changes in their home environment. It also allows caregivers the opportunity to identify other potential problems, learning needs and coping skills needed prior to discharge.
Going Home
When the rehabilitation team determines the patient is ready for discharge to outpatient or homebound therapy, team members help patients prepare for this transition. Through the discharge planning process, patients and their families will receive educational services and specific home care instructions as well as referrals and follow-up consultations.
Second Time Around Program
The Baldwyn Nursing Facility supports a Second Time Around Program which makes new and used medical equipment available to patients who may not have the funds to purchase needed items.
