Population Health
Managing chronic diseases can be stressful and confusing. Our population health staff works in partnership with you and your providers to teach you to better manage your health conditions. We can help coordinate services across several providers, and also help you navigate from an acute hospital stay or extended skilled nursing facility stay back to your home.
We help you make sense of your medications, ensure you have the follow-up appointments that you need, and help coordinate community resources and education for you.
Some of the conditions we can help with include:
- High blood pressure
- Diabetes
- Heart failure
- Chronic lung disease (asthma or COPD)
Our health management programs provide:
- Tobacco cessation
- Weight management
We will teach you how to monitor your condition, understand the influence of nutrition and exercise, adhere to medication prescriptions and know when to contact a provider.
Population Health staff works with patients in one-on-one sessions (by phone, in person or virtually), to set personal goals and complete an action plan that will help you make the best decisions about your health.

- Social worker
