Who Qualifies for Home Health?

If you have trouble communicating, are dependent on others to take you to doctor's appointments, or if it is particularly taxing or you feel unsafe leaving home, Home Health may be a good fit. Home Health can provide care for those recovering from recent conditions and those diagnosed with chronic conditions.

Abdominal wound

Can include traumatic injuries or post-surgical care

Assistance for transitioning home after being discharged from the hospital

Can include serious illnesses like pneumonia, flu or infections that need IV treatment
