2. Electronic Order Submission FIRST, then call 1-866-912-1486 to schedule.
3. Fax Order Submission FIRST to (662) 377-6777, then call 1-866-912-1486 to schedule.

Information REQUIRED for Correct Order Submission:

• Date of order
• Patient’s last name, first name and middle initial
• Patient’s date of birth
• CPT code of test or procedure, including description
• ICD-10 code, including description
• Name and signature of PROVIDER creating order. (Signature stamps and nurse’s signatures are not allowed)
• Insurance information
• Guarantor for minors

Additional information preferred, but not required:

• Demographic information (address, phone number, etc)
• Primary care physician
• Authorization number
