Medical Staff

Lakesha Butler, NP-BC: Earned her associate's degree in nursing from Mississippi University for Women in Columbus and her bachelor's degree in nursing from the University of Southern Mississippi in Hattiesburg. Butler completed her master's degree at the University of Alabama at Birmingham.

Leslie Hale, NP-BC: Earned her associate's degree in nursing from Itawamba Community College. She completed her bachelor's degree in nursing at Mississippi University for Women, where she also completed her master's degree and her family nurse practitioner certification. She is also a certified diabetes educator.


Robbie Jones, NP-C: Earned an associate of arts degree from Itawamba Community College, then completed associate and bachelor degrees in nursing from Mississippi University for Women. She also completed her nurse practitioner training at MUW. Jones earned certification for advanced diabetes management from the University of Alabama at Birmingham. She is certified in Basic Life Support and Advanced Cardiovascular Life Support.


Tandalaya Traylor, CFNP: Completed her bachelor's degree in nursing at the University of North Alabama in Florence and her master's degree at Delta State University in Cleveland, Miss. She is a recipient of the 2014 Top Performer in Diabetes Mellitus Award for the NMMC clinic system. She currently serves as an American Diabetes Association (ADA) Education Recognized Program Auditor, ADA program coordinator for the Diabetes Treatment Center and a member of the ADA organization.

The American Diabetes Association has recognized NMMC as a Diabetes Center of Excellence.

Office Hours

Our clinic is open Monday-Thursday 8 a.m.-5 p.m. and Friday 8 a.m.-noon.


Appointments may be made by calling (662) 377-2500 during office hours. Every effort will be made to provide the earliest possible appointment for the convenience of the patient. Due to the unscheduled nature of emergencies, occasional delays do occur. We hope you will understand these delays are unavoidable.

If you are unable to keep your appointment, please cancel as far in advance as possible. Another patient who can be booked into the open time will be grateful for your thoughtfulness.

The Diabetes Treatment Center offers diabetes treatment and consultation for all ages, including:


  • Inpatient and outpatient medical treatment
  • Continuous glucose monitoring
  • Assistance with specific skills, such as foot care, meal planning & sick day management
  • Diabetes management before and during pregnancy
  • Insulin instruction
  • Insulin pump initiation
  • Oral medication management
  • Medical nutrition therapy
  • Pediatric diabetes program
  • Exercise recommendations
  • Community programs
  • Educational seminars
  • Individual and family support
  • Financial assistance

Inpatient Services

The staff teaches hospitalized diabetes patients survival skills such as monitoring, managing hypoglycemia and insulin administration.

Outpatient Services

In-depth education including individual nutrition counseling, self-management and insulin pump initiation is provided in an outpatient environment.

Comprehensive Diabetes Self-Management Program

This class includes intensive diabetes management covering every aspect of managing diabetes on a daily basis. The class is taught at NMMC hospitals in Eupora and Tupelo.

Support Services

Living with diabetes is a daily challenge. The Diabetes Treatment Center staff offers a variety of follow-up services to help patients and their families stay informed, up-to-date and in control.

Healthy Cooking Class

The Diabetes Treatment Center offers "Healthy Cooking for People with Diabetes," taught by a registered dietitian. Sessions include a cooking demonstration and recipe sampling. Participants learn how to modify recipes to decrease the sugar and fat and how to incorporate portion control and carbohydrate counting into meal planning. Other topics include food label education and shopping tips.

Diabetes Assistance Fund

The daily battles of diabetes can be stressful enough without the added burden of financial pressures. The Diabetes Assistance Fund provides supplies and self- management training for patients who are financially unable to control their disease. Administered by the Diabetes Treatment Center and the Health Care Foundation of North Mississippi, the fund's goal is to improve quality of life and to decrease preventable hospital admissions for people with diabetes. Funds are designated for patients who meet eligibility requirements.

Community Outreach

Diabetes Treatment Center staff members are actively involved with community education, health fairs and fund-raising efforts for local diabetes patients and research toward a cure.

Patient Portal Offers Secure Online Services to Your Health Information

We now have the ability to exchange secure online correspondence with our patients and keep your information up-to-date. If you have Internet access and sign up for this service, you will have the ability to do the following at your convenience:

· View your patient chart summary

· Request an appointment

· Request a medication refill

· View test results

· Change personal information

· Access forms

To access the service, you will register using a PIN (Personal Identification Number) that will be provided by our clinic staff to you in a personalized letter after a visit to our clinic. The PIN will expire in 30 days. If your PIN has expired before you access your account, please visit us in person to obtain a new PIN. To ensure patient confidentiality, we do not provide this information over the phone.
